Does anyone have any suggestions on how to remove a thread adapter that's been put in upside down?

Needle nose pliers - clamped em on one edge of the adapter and lever against the opposite side to hold the adapter in place while you twist the two bits apart.

i do the above and hold my tongue just right then eventually when that doesn't work i find a really wide screwdriver jam it in and unscrew and throw away the insert , every mic in my place is running the one size on all the inserts are loktighted in so it can't come out
so this shit never happens again

In the future, throw away any adaptors which have a slot on one end only and make sure you get and use those with slots in both ends.
