Excessive noise floor.

I've got major hiss problems with an MPX500 that I recently purchased. Gain structure is set correctly. Other than inherent problems with circuit design, what can cause excessive noise and can it be fixed?

I also have the same (but nowhere near as bad) issue with a DR260.

Using reasonable or good quality balanced cables? (i.e. not the cheapest, crappiest, poorly terminated crap that is available these days!) Is it located near anything that is electrically noisy? Some gear is susceptible to noise on the mains, have you tried it in another studio, or with a mains noise filter?

Hope I'm not overstating the obvious, but its easy to forget something simple!

All balanced and running through a power conditioner.
